SERVICES

OUR SERVICES

Passionate prevention with a sealed promise.

Light Housekeeping

Routine house cleanings promote a healthy living environment. When household chores become difficult, our light housekeeping services can help give your loved ones a sense of well-being and independence. Our caregivers will ensure that their home is neat, organized, sanitary and safe by helping with dusting, vacuuming, sweeping, mopping, dishes, bathrooms, laundry and more.

Passion Safety

One of the most important concerns about independent living is safety. Even a small trip hazard can cause a life-threatening event to happen. Our caregivers specialize in incident prevention to keep your loved ones comfortable and safe. We can assess your home and help manage potential dangers such as trip hazards, medical waste, contaminants, medication routines, and dietary hazards.

Medication Management

Some medication regimens can be difficult to follow and become dangerous if not taken correctly. Our detailed medication management process ensures that your reminders are always on time with precise medication and dosage information. We also track and monitor the effectiveness of prescribed regimens so that you and your loved ones get the desired outcomes by managing prescription drugs, over-the-counter medicines, dietary supplements, and pharmacy medicines.

Hospice Care

We have a deep passion to provide care for hospice diagnosed and homebound clients including people living with severe illnesses. Our hospice care is compassionate and gentle. We focus on providing relief from the stress of coping with illnesses by monitoring serious illnesses by providing palliative care, intermittent care, continuous care, emotional and spiritual support.

Physical Rehabilitation

We strive to improve your loved one’s physical functions through rehabilitation and support. Our treatment plans can help reach physical goals and optimize musculoskeletal health. We'll provide your loved ones with personalized rehabilitation in a friendly and supportive way that can help with post-operative rehabilitation, arthritis, herniated discs, osteoporosis, muscular pain, carpal tunnel and more.

Care Coordination

Successful communication is essential to the safety of your loved ones. We collaborate with your physicians and support systems to ensure consistent optimal care is always provided. This reinforces our strong culture of safety within our practice and includes plans of care, medical documentation, physician awareness, coordinating services, appointments, transportation and more.

Pain Management

When it comes to managing pain, every person is different. Therefore, our treatment plans are tailored to meet the specific needs that your loved ones may require. Our goal is to help clients with chronic pain achieve maximum comfort and independence. We use various methods to reduce and control pain to include pain evaluation, pain management, symptom control, proper repositioning and alternative measures.

Wound Care

Wounds that don't heal with standard care can be problematic and confusing. We work with your loved ones and their doctors to evaluate conditions and create individualized wound treatment plans. Our nurses are certified to treat a wide range of chronic and recurrent wounds such as non-healing, acute, pressure sores, ulcers, surgical and radiation related.

Dietary Assistance

Good meal plans ensure that your loved ones receive the nutrition they need to thrive. We provide compassionate, patient-centered nutritional therapy for a variety of health conditions. Our caregivers always follow proper food handling procedures to maintain health and safety. Our dietary assistance includes meal prep, monitoring and recording consumption levels, nutritional information, prescribed meal plans, maintaining dining and food prep areas, and food safety compliance.

Monitor Vitals

A big part of caring for your loved ones at home is consistently monitoring their vital signs throughout the day. Tracking changes is equally important for accurate communicating with physicians. We are certified to monitor and track all major vital signs such as heart rate, blood pressure, temperature and respirations.

EKG & Holter Placements

EKGs and Holter monitors look for abnormalities that affect normal heart function. Early detection is critical, and our nurses are certified to perform these tests and track the results. Your healthcare providers may need to monitor heart related symptoms such as chest pain, tiredness, shortness of breath, dizziness, irregular heartbeats and pacemaker malfunction.

Patient and Caregiver Education

Loved ones that understand their health are more likely to help treat their health. Accurate and thorough education allows our clients and their care team to understand conditions and treatment options. We develop educational solutions geared towards increasing and improving medical outcomes such as patient engagement, patient satisfaction, understanding of medical conditions, treatment follow through, compliance and more.

We're always here to help.
